• G12 Report Card (1st & 2nd Sem Original & Photocopy)
• Good Moral Certificate (Original Copy w/ Dry Seal)
• PSA Birth Certificate (2 clear photocopies)
• 2pcs. 2x2 picture with nametag (any formal background)
• 2pcs. Long, Expanded Envelope

A. For Criminology Students
- Medical Certificate (Original & Photocopy)
- Drug Test Result (Original & Photocopy)
B. For Married Femaale Students
- PSA Marriage Certificate (2 photocopies)
- LCR Marriage Certificate (2 photocopies if PSMAC is not clear)

• Certificate of English Profeciency from the school last attended
• Copy of Bureau of Immigration and Deportation Registration/Issuance of Identification Certificate (for Dual Citizen)
• Alien Certificate of Registration (ACR) authenticated by Philippine Foreign Service establishment located in the student's country of origin.
• Birth Certificate authenticated by Philippine Foreign Service establishment located in the student's country of origin.
